To apply for long term disability benefits, please complete the Long Term Disability Claim form.
The Benefits Administrator will complete the Employer’s Statement and the Employee will then complete the Employee’s Statement, have their physician complete the Physician’s Statement, and forward to the insurer. Application should be made approximately 6 weeks prior to date that LTD benefits could begin, to ensure all of the required information is in place on a timely basis.
